About the Service
Ty Wyth is a purpose-built long-term solo placement for one young person with a physical disability, supporting up to age 18.
The environment provides positive, reliable, and consistent care with clear boundaries, within a supportive 2:1 support ratio, guided by our team including the Registered Manager and Team Leader.
About the Role
As a qualified or progressing towards a level 3 in Health and Social Care, you will lead a team of support workers, ensuring high standards of care and personalized support, including supervising staff, case management, and service development.
How You'll Create Brighter Futures
* Lead shifts to provide high-quality service
* Coordinate responsibilities, liaise with multi-agencies, and produce reports
* Attend professional meetings such as reviews and court sessions
* Supervise and guide staff according to policies and procedures
Let's Talk About You
* Level 3 qualification in Health and Social Care for children and young people or recognized equivalent, or working towards it
* Experience leading a team of support workers
* Flexibility to work various shift patterns on a rolling rota
* Safeguarding training and experience
* Driving license for transportation (desirable)
