An opportunity has arisen within the Community Mental Health Team, Old Age Psychiatry at The Meadows for a Qualified Nurse Associate to join our team.
You will be a member of a multidisciplinary team who work with service users whose lives are affected by organic and/or functional mental health problems.

Responsibilities:
1. Provide support & promote recovery to older people experiencing mental health/psychological distress due to acute physical health or social change.
2. Assist nurse practitioners in assessment, planning, implementing, and evaluating individual care plans.
3. Ensure that service users engage effectively with the agreed Plan of Care.
4. Offer advice and support to non-mental health staff when dealing with patients with mental health problems.
5. Provide regular & practical support for service users and their carers in managing dignity and independence.
6. Promote a positive attitude to the care of service users.
7. Support with activities of daily living.
8. Maintain service users’ needs at the forefront at all times, promoting independence and choice.
9. Maintain adequate records according to Trust Record Keeping Standards, entering appropriate details on service users' case notes as necessary.
10. Attend and actively participate in training sessions, team/care planning review meetings, and supervision as appropriate.
11. Contribute to the ongoing development of the service and actively participate in individual professional and personal development.
12. Undertake other duties as determined from time to time within the general scope of the post and adhere to trust policy and procedures.

The role of the Nursing Associate is to work with the multi-professional team and the Registered Nurse contributing to the ongoing assessment, providing and monitoring care in line with the service pathways and as agreed with supervising clinician(s). They will monitor the condition and health needs of people within their care on a continual basis in partnership with the multi-professional team. The Nursing Associate has a breadth of knowledge across the lifespan, providing holistic and person-centered care and support for people of all ages and in a variety of settings. The Nursing Associate will help to bridge the gap between Health Care Assistants and Registered Nurses by working independently under the leadership of Registered Nurses, working within the sphere of nursing and care and within all aspects of the nursing process. Nursing Associates are equipped with the knowledge, understanding, skills, attitudes, and behaviours relevant to employment; and will act in accordance with the NMC Code: Professional standards of practice and behaviour for nurses, midwives, and nursing associates.
