Proactively identify and work with people, including the frail/elderly and with long term conditions Provide navigation of care and support across health and care services Focus on supporting and delivery of personalised care to reflect local PCN priorities, health inequalities or at risk group patients Ability to run searches in the clinical system for meeting target specific goals and objectives Ensure patients are contacted and directed to the right service based on practice call/recall policy and clinical system alerts Job Responsibilities To assist in the monitoring of the practices performance against QOF & IIF compliance on a monthly, quarterly and annual basis To assist in the monitoring of compliance with local and national enhanced Services, Vaccination & recall programs Dipping urine, taking blood pressure, phlebotomy and other observations and clinical tests Extracting all information from clinical letters that needs coding and adding to notes Confidentiality: While seeking treatment, patients entrust us with, or allow us to gather, sensitive information in relation to their health and other matters.
