Job summary The States of Guernsey are looking for a Staff Nurse to join the team at Community Health & Social Care. The post holder will assist the Senior Team Leaders in assessing, planning, implementing and evaluating programmes of care for Service Users living at home within a defined caseload. The postholder will also assist the Senior Team Leaders with the management of the caseloads, day-to-day support, supervision, and deployment of staff. The post holder will provide high quality Social Nursing Care supporting Service Users in the community and will work as part of a multi-disciplinary team within an integrated Social/Nursing care model.
