Experienced in planning and resource maximization, able to negotiate with customers to meet their needs. Proven ability to work within a multi-disciplinary team, liaising with families, carers, and staff of various agencies. Capable of managing workload efficiently, prioritizing tasks, and adapting to changing demands. Expertise in implementing and monitoring quality standards, able to perform assessments in compliance with relevant regulations. Experience in delivering services using computerized systems and working with vulnerable adults. Knowledge of legislative framework and Health and Social Care integration. Preferably a car owner/driver. What the Job Involves Support and deliver the development of service provision using advanced skills, knowledge, and understanding of practice within the area of specialism. Promote independence, choice, and control for service users through quality assessments and service delivery. Implement safeguarding and health and safety policies, chair complex strategy meetings, and manage staff through the process. Manage, monitor, and review care plans in collaboration with users, carers, and other agencies. Develop and manage systems associated with team functions, including data recording and analysis. Actively encourage customer participation and initiate research for service improvement within the specialist area. Perform comprehensive assessments and care planning for individuals of all ages and disabilities, considering carers' needs. Recommend and arrange for the provision of specialist equipment and major adaptations involving various stakeholders. Participate in flexible work options, including hybrid working, and receive regular supervision. Compliance with Council's Policies, Health and Safety, and safeguarding of children and vulnerable adults.
