You will be part of the Allied Health Professions (AHP) undertaking an Physiotherapist role within the South Falls Service providing specialist assessment to patients who have fallen. This includes providing advice and onward referral for therapy/diagnostics. This assessment work is predominantly provided within patient's homes therefore the ability to travel across the Staffordshire area is essential. Main duties of the job Working on your own as a registered practitioner within the established multidisciplinary team you will undertake a community Occupational Therapy role for the South Falls Team, supporting the other specialist teams as demand requires. You will be expected to assess, identify and agree goals and treatments plans using diagnosis and evidence based treatment programmes to meet a wide variety of patient needs in the most appropriate location in the community e.g. patients' own homes, clinics, care homes and day centres. The role will involve supporting the team to provide training on falls prevention on an ad hoc basis at the request of Clinical/Ops Lead.
