Job summary
To providemedical services to patients presenting with acute needs, ensuring the higheststandards of care, in accordance with the practices Personal Medical Services(PMS) contract. The role will also include health promotion, family planningand sexual health. The role holder should be able to prescribe and will be ableto reach safe clinical decisions within professional boundaries on anautonomous basis.
Main duties of the job
Makeprofessional, autonomous decisions in relation to presenting problemsincluding history taking,examination, investigation, diagnosis, treatment and referral where appropriate, whether or not that patienthas already been triaged by the practice. To follow up such patients as isnecessary.
To provide theabove not only in 10 minute face to face consultations but also to undertaketelephone consultations and home / residential home visits.
To manageacute exacerbations of patients chronic diseases.
Contribute tothe development of and adhere to protocols and care plans for themanagement ofchronic disease and common medical conditions.
Provideappropriate preventative healthcare, counselling, education and advice.
Prescribe andreview, as necessary, in adherence to the practices prescribing formulary.
Produceaccurate, contemporaneous and complete records of patient consultation,consistent with legislation, policies and procedures and to maintain the utmostconfidentiality in relation to such records.
Supportpatients to adopt health promotion strategies that promote healthy lifestyles,and apply principles of self-care.
Utilise anddemonstrate sensitive communication styles, to ensure patients are fullyinformed and consent to treatment
Work in thecontext of a multidisciplinary team with a collaborative, supportive and holistic approach, including referralto, and liaison with hospital services or other agencies.

1. Registered Nurse with Nursing and Midwifery Council
2. Recognised Nurse Practitioner qualification/relevant nursing/health degree.
3. Meets NMC revalidation requirements in accordance with the NMC

6. Minor illness qualification
7. Evidence of CPD
