Requirements:
* Join a newly created all-age adult mental health hospital discharge team, supporting older and working-age adults with mental health diagnoses.
* Collaborate with Pennine Care colleagues to improve mental health services and outcomes for individuals leaving the hospital.
* Ensure safe and timely discharge from hospital for individuals with functional or organic mental health conditions.
* Work collaboratively with individuals, carers, health professionals, advocates, and third-sector colleagues.
* Co-produce Care Act assessments, support plans, and risk assessments.
* Conduct Mental Capacity Assessments and chair Best Interests Meetings.
* Contribute to improving the discharge process through teamwork and partnership.
What we are looking for in a Social Worker:
* Social Work England registration
* Eligible to work in the UK
* Hold a full UK licence
* Post-qualified experience
* Degree level or equivalent in Social Work
