Main duties of the job We are looking for a dedicated and compassionate Community Palliative Care Nurse to join our team, managing a caseload of patients and their families or caregivers with palliative care needs. In this role, you will assess, develop, and implement individualised care programs tailored to the unique needs of each patient. Key Responsibilities: Manage a caseload of patients and their families or carers with palliative care needs, assessing, developing, and implementing comprehensive care programs Provide expert palliative care advice and support to patients, caregivers, and healthcare professionals, utilising your specialist knowledge to address key issues such as treatment, pain and symptom management, psychosocial needs, and ethical dilemmas. Collaborate with other Palliative Care Nurses as necessary Assist patients in achieving self-care when appropriate and educate caregivers to enable them to provide effective support at home or in hospital settings Establish and maintain highly effective communication with patients, caregivers, and professionals across health and social care, developing therapeutic relationships to navigate sensitive, distressing, and complex issues Act as an advocate for patients, families, and caregivers when necessary, ensuring that their needs and wishes are prioritised.
