Job summary The post holder will lead on children's Continuing Care assessments and reviews to assess whether a child's needs are such that they meet the criteria for full NHS continuing care. The post holder will be expected to undertake NHS Continuing Care assessments, comprehensive nursing assessments, contribute to the completion of EHC plans and take the lead and co-ordinate a multidisciplinary team when completing Continuing Care documentation. The post holder will provide professional and clinical leadership and be a source of expertise on matters relating to children's Continuing Care processes for the ICB, Local Authority, healthcare providers and other local agencies and organisations. The post holder will be responsible for delivering assessments of individual's eligibility for Continuing Care and case manage existing patients. The post holder will ensure that assessments and reviews are carried out in a timely manner according to national specifications. Main duties of the job The post holder will work with all stakeholders including patients and their families, to ensure that funding decisions are taken in a transparent and coherent manner. The post holder will keep accurate records and be able to demonstrate that funding recommendations are evidence based and made according to the criteria described within national CHC frameworks.The post holder will have extensive knowledge and experience of working with children and young people with complex health care needs both in the acute and the community settings. You will be responsible for a portfolio of complex cases with a specific view of ensuring that care packages are responsive to individual's needs as assessed. The role will require excellent communication skills and the ability to work supportively and effectively with service users, their families and representatives in frequently challenging and contentious circumstances.
