Salary: £30,420 - £37,030 per annum (pro rata if part time), Salary period: Yearly, Grade: (Band 5).
Job Overview
We have an exciting opportunity for a Band 5 Community Nurse to join our Community Nursing Team based in Cardigan, Ceredigion. The role will allow you to support the management of a delegated caseload within the Community Nursing Team providing high quality and complex care management to patients in their own homes, with the guidance and support of an experienced District Nurse Team Leader.
The post holder will ensure high standards of care are delivered to patients and their families/carers within the home or clinic environment. This includes liaising with staff, patient/carers and other agencies/organisations as required to ensure appropriate delegation of duties and risk management, whilst ensuring effective communication between those involved in the care package.
This is a permanent position, worked over a 7-day period including weekends and bank holidays covering an 8am-8pm service. Various full and part time hours are available. The post holder will need the ability to travel between sites in a timely manner. The ability to speak Welsh is desirable for this post: English and/or Welsh speakers are equally welcome to apply.
Qualifications And Knowledge
Essential criteria:
* Level 1 Registered Nurse (Adult)
* Current NMC Registration
* Evidence of personal continuing development, statutory and mandatory training
* Awareness of Health Board policies, procedures and guidelines
* Ability to relate theory to practice
* Understanding of primary health care working
Desirable criteria:
* Post-registration experience, in acute services
* Teaching and assessing
* Understanding of clinical governance, health & safety, risk management awareness
* Community experience
* Willing to undertake further training as necessary
* Evidence of post-registration education Diploma/ Degree Level
Experience
Essential criteria:
* Ability to demonstrate sound knowledge of NMC Code (2018)
* Able to relate theory to practice through reflective skills
Desirable criteria:
* Relevant community experience
* Ability to demonstrate sound knowledge of the community
Language Skills
Desirable criteria:
* Welsh Speaker (Level 1)
