You will guide the coordination of Care Home MDTs and facilitate weekly rounds with the GP practices ensuring that residents in need of a review or a discussion are identified and prioritised. You will collate information to help individuals prepare for, clinical discussions with primary care professionals. Working closely with Care Home Leads and other primary care professionals within the PCN, you will complete the administration for a caseload of care homes, ensuring timely assistance for staff and residents, and addressing changing needs. You will also support the implementation of local priorities, address health inequalities, and promote population health. This role is designed to improve communication between Primary Care and care home staff, aiming to prevent unnecessary GP visits and hospital admissions. The post holder must be adjustable, as the role will evolve alongside the Enhanced Health in Care Homes Framework initiative. The role will involve providing administration support for the Social Prescribing Team, supporting education of non-clinical and clinical staff within their PCN multi-disciplinary teams on what other services are available within the community and how and when patients can access them.
