Hours: 37 hours x 4 positions. 2 Permanent, 2 Temporary
Contract: Permanent x 2 positions & 12 Months Fixed Term x 2 positions
The advertised salary includes a supplement (JWC) to the base grade of the post. JWC supplements are conditional on the physical demands and/or working conditions of the role.
An exciting opportunity has arisen for four highly motivated individuals to work as part of our brokerage team. Two of the positions will be permanent and two positions are fixed term for 12 months, but may be extended.
About this opportunity
Brokerage is a key part of the successful delivery of Adult Social Care. If successful you will support the delivery of excellent services to a wide range of internal and external stakeholders such as: People who use adult social care services, social workers, commissioners, providers of adult social care and health partners.
The role will include:
* Supporting people with eligible adult social care needs to access the services they need by working with providers of adult social care.
* Working with colleagues to ensure peoples’ identified care needs are met in the most appropriate way.
* Supporting the brokerage service to contribute to ensuring the delivery of care is effective and safe.
* Making sure information is recorded accurately and in a timely manner to ensure quality management information and essential data is up to date.
* Building effective relationships with a variety of stakeholders.
* Using a range of specific IT systems in order to carry out the functions of the role.
