Job summary An exciting opportunity is available for a band 5 nurse assessor within the team which is based at Laurence House in Catford. The role encompasses assessment of Nursing Home patients for eligibility for Funded Nursing Care and completion of the CHC checklist. This provides an opportunity to work with patients and relatives and also with the homes to promote quality and safety of care provision. There is a full induction and training programme with opportunity for development to a band 6 CHC assessor role. Main duties of the job This post is part of the Continuing Healthcare Team, a multidisciplinary health and social care team which sits within Lewisham's Integrated Commissioning Team hosted by Lewisham Council. As a member of this team the post holder will: o Assess and determine whether individuals living at home or in supported care settings are potentially eligible/ remain eligible for NHS-Funded Nursing Care (FNC) or trigger (re)assessment for fully funded healthcare (CHC). o To review packages of care as may be required to ensure that the person's health needs are being well met. o Support the wider service quality intelligence network (health, social care and third sector) to advise on the quality of care provision and how it might be improved.
