The ICT is a multi-disciplinary team thats aim is to prevent avoidable hospital admissions and reduce delayed hospital discharges by assessing needs and providing rehabilitation with/without care support in the home setting. As part of this multi-disciplinary team, the post holder will manage their own caseload and work alongside colleagues to ensure seamless service delivery. The post holder should be able to demonstrate evidence-based skills in assessment, planning, implementation and evaluation of individualised treatment plans and will be responsible for the delegation of work and supervision, training and development of students.
