About the Role
Barrack Lane Medical Centre has a fantastic opportunity for a practice nurse to join our team on a part-time basis for 27 hours per week, Wednesdays, Thursdays, and Fridays.
The main responsibilities of the role include:
1. Assess, plan, develop, implement, and evaluate treatment programmes that promote health and well-being, and evaluate individual treatment plans for patients with known long-term conditions.
2. Work with other health care professionals to diagnose, monitor, manage, and treat long-term conditions.
3. Review medication for therapeutic effectiveness, appropriate to patient needs and in accordance with evidence-based practice and national and practice protocols.
4. Work with patients to support adherence to prescribed treatments.
5. Provide information and advice on prescribed or over-the-counter medication regarding medication regimens, side-effects, and interactions.
6. Prioritise health problems and intervene appropriately to assist the patient in complex, urgent, or emergency situations, including initiation of effective emergency care.
7. Implement and participate in vaccination and immunisation programmes for both adults and children, including patients travelling abroad.
8. Meet the needs of patients presenting for opportunistic wound care.
9. Perform investigative procedures including venepuncture, ECG recording, spirometry, audiometry, and 24-hour ECG or BP measurement.
10. Maintain accurate patient computer records at all times on SystmOne.
About the Candidate
All applicants should have the following qualifications, registrations, skills, and experience:
1. A relevant nursing degree.
2. Current NMC registration with an active PIN.
3. Fundamentals of Practice Nursing course (General Practice Nurse Foundation Programme) or equivalent experience or qualifications.
4. Competence to undertake asthma, COPD, and diabetic clinics and reviews, holding relevant certificates.
5. Evidence of clinical skills and experience in treatment room consultation, phlebotomy, ECG recording, screening, management of long-term conditions, cervical cytology sampling, vaccinations and immunisations, assisting with minor surgery, dressing and wound care, and recording spirometry.
6. Compassionate manner with an empathetic approach and demonstrable kindness in all aspects of work. The successful candidate will value the dignity of our patients, their relatives and carers, as well as their colleagues at the practice.
7. Knowledge and experience of its application in supporting long-term conditions.
8. Insight and understanding of current issues in nursing and the wider NHS.
About Us
Barrack Lane is a busy practice in the town centre of Ipswich, Suffolk, with a practice population of over 20,000 patients. The practice is based on two sites, the main surgery in the centre of the town, and a branch surgery at Stoke Park.
Our clinical team consists of GP partners, salaried GPs, advanced nurse practitioners, advanced care practitioners, dieticians, paramedics, physicians associates, pharmacists, pharmacy technicians, and a practice nursing team and HCAs. This team is supported by a strong administrative, coding, and care-navigating team.
Barrack Lane is part of a PCN (Primary Care Network) with Ivry Street Medical Centre in Ipswich, covering a population of 32,000 patients. The PCN allows the practices to share resources and offer enhanced services to our patients, employing care coordinators and social prescribers to work with patients, especially our vulnerable patients, to help them access the best care.
