ClinicalThe post holder will:Work as an autonomous practitioner managing a caseload in an effective and productive manner.Develop links with primary and secondary health care services in order to maintain effective care pathways, and compliance with local and national strategy.Actively promote service user and family involvement in complex care planning.Undertake screening assessments in determining eligibility for the service and ensure any unmet or ineligible needs are forwarded to the relevant agency.Assess and manage complex clinical issues in relation to service-users; can advise and collaborate with carers, professionals and other agencies around complex clinical interventions and outcomes.Identify and evaluate specific service users needs through specialist assessment including identifying where a standard or enhanced response is required.Work within a Positive Behaviour Framework to assess, monitor and formulate positive behaviour support plans that aim to improve quality of life for the service user and their carers.To ensure the effective application of the Mental Capacity Act.Ensure nursing assessments include consent, safe administration and medication monitoring and reporting.Use a range of specialist assessments to identify needs for service users including both physical and mental health or other complex needs and to support and mentor other nurses development in this area.To ensure the process of referral, assessment, planning, intervention and evaluation of care is efficiently and effectively delivered with clear quality outcomes. This includes implementing first and/or urgent assessments as requiredTo develop a plan of care as appropriate to meet the health needs of the person with learning disabilities.Identify service user related training required by other agencies in order to ensure appropriate person-centred care delivery facilitating access to this internally or via links with external NHS colleagues or providers.Contribute to safeguarding investigations as required.Utilise evidence-based practice, policies and procedures and the management of risk.Make clinical judgements based on specialist knowledgeAdhere to own professional code of conduct/practice and all organisational policies and procedures.Be responsible for prioritising own workload and delegating to/supporting others.Demonstrate ability to work as part of a team but is also competent and confident to work independently, providing expert advice to all allied health professionals.Take the lead in organising and chairing multi-agency meetings relevant to complex physical and emotional health needs.Develop, implement, and review risk assessments for all clinical work-based activities including those with service-users.Contribute to planning and organisation within Community Learning Disability Service.Participate and on occasions take the lead on good practice initiatives in consultation with the Clinical Nurse Specialist.To implement clinical and operational policies and procedures. Managerial Act as an effective role model by demonstrating professionalcompetence.To deputise for the team manager / clinical nurse specialist as required.To provide supervision, including caseload management, and appraisal to more junior staff.To participate in the recruitment of staff.To participate in monthly managerial and clinical supervision with the Team Manager and Clinical Nurse Specialist respectively.To identify service deficiencies and offer options for intervention/problem solving in collaboration with the Lead Learning Disability Nurse/Integrated Team Manager.To ensure equipment and clinical records are stored and maintained correctly.Education, Training and DevelopmentTo be responsible for organising and delivering training events.Participate and on occasions take the lead on good practice initiatives with support from the Team Manager / Clinical Nurse Specialist.To contribute to service development initiatives. To support junior staff regarding the implementation of service developments and improvements.To promote best practice in supporting people with a learning disability who have complex physical and emotional health needs. To contribute to the development of tools/resources/service policies which meet divisional, trust and national objectives for example CQC.
