Beechfield Medical Centre is a well-established and highly regarded practice based in the heart of the Lincolnshire market town of Spalding. We operate from modern purpose-built premises and we are now looking to strengthen our team further with the appointment of a Nurse Practitioner or Advanced Nurse Practitioner as we look to develop and enhance the services we are able to offer to our 19,500 patients, particularly as we integrate our services for the wider community of a Primary Care Network within our locality.
This is an exciting role where proactivity, initiative, and a can-do approach to service development will be as important as the professional experience and range of clinical skills that you can offer the Practice. Working with Lincolnshire Training Hub, we will support you through the Paramedic Preceptorship Programme which will equip you with all of the necessary skills to succeed in this role.
Main duties of the job
Duties will include:
1. Assess, diagnose, treat, refer or signpost patients who attend the surgery with undifferentiated or undiagnosed conditions relating to minor illness.
2. Undertake home visits, primarily for acute conditions, as assessed by the Duty Doctor, and within the boundaries of professional qualifications.
3. Use advanced clinical skills to provide education to patients, promoting self-care and empowering them to make informed choices about their treatment.

Job responsibilities
Duties will include:
1. Provide assessment, diagnosis, and treatment at first point of contact by attending to patients according to their needs either over the telephone, in the Practice, or patients' homes.
2. Support and continue the development of an on-the-day emergency patient service in the Practice.
3. Accurately prioritise patients, including in emergency situations, demonstrating the use of a variety of techniques to elicit the history of an event/illness, including past medical and drug history.
4. Evaluate clinical information from examination and history taking, and initiate appropriate treatment and/or referral.
5. Undertake and interpret diagnostics and take appropriate action to manage patient care, whilst distinguishing between abnormal and normal findings in order to relate them to a possible diagnosis.
6. Signpost appropriately to other services in the identified healthcare pathway.
7. Refer patients to appropriate care settings, within locally agreed healthcare pathways.
8. Discharge or treat, as appropriate and within the scope of practice.
9. Maintain accurate and systematic documentation of patient interactions and clinical care provided, including adverse events as per practice policy.
10. Advise and support members of the public on healthy lifestyles, health promotion, and disease prevention including where possible data collection for QoF during patient contact.
11. Provide clinical advice and facilitate learning to other members of the clinical team where appropriate.
Person Specification
Experience
* Ability to manage and prioritise workload.
* Proven ability to work autonomously and as part of a team.
* Previous experience of non-medical prescribing and using PGDs.
* Experience of the ECP/Advanced Practitioner role within Primary or Secondary Care.
* Current knowledge of government targets and initiatives and N.I.C.E. guidelines and their impact on clinical practice.
* Understanding of the N.M.C/H.P.C Code of Professional Conduct.
* Knowledge of child protection issues.
* Understanding of clinical governance and its significance and impact upon practice.
* Basic IT skills in word processing and e-mail.
* Experience in using I.T based patient documentation systems.

Qualifications
* Registered Practitioner with acute and minor illness management qualification and experience at urgent care/GP surgery settings.
* Already working or experience at Primary care/Urgent care setting and confidently able to deal with acute and minor illness management.
