An exciting opportunity has arisen for a clinician to join the Intensive Support Team (IST)
The IST is for people with learning disabilities who are at a significant time in their lives when they need additional support in community.
Many of the people we support have experienced trauma, they may have additional mental health needs, sensory processing difficulties & communication needs. They may be accommodated in services who struggle to meet their complex needs.
The Teams aim is to provide dynamic support to prevent an admission to hospital. We aim to keep people safe, empower people to overcome challenges & to provide them/their staff team & or carers with the skills and knowledge needed to overcome the present challenges they face.

Main duties of the job
You will be responsible for delivering high-quality clinical care and ensuring all work is needs led, evidence based and in line with current legislation.
Undertake detailed clinical assessments and following assessment derive a formulation and plan of intervention, ensuring this is negotiated with the service user and their carers (where appropriate).
To promote, establish and maintain effective communication with service users who have a range of communication difficulties and impairments
Undertake risk assessments and outline intervention plans for people with complex needs.
Provide clinical supervision and appraisals of staff within the team and supervise students while on placement with the team.
To work with partner agencies to facilitate inclusion and improve health and social care outcomes.
To maintain accurate and up to date records for all service users in line with Trust policy and National Legislation and Guidance.

Essential
1. Current professional registration with an approved professional body
2. Evidence of Continuing Professional Development
3. For registered nurses: MIP, FLIP, ENB 998 Teaching & Assessing in Clinical Practice or City & Guilds 730 Teaching in Adult Education. Must be achieved within agreed timescale
4. 18 months post qualifying experience including sign off of appropriate preceptorship or competency framework
5. Recognised sign-off mentor, clinical educator or equivalent. Must be achieved within agreed timescale
6. Key skills in literacy, numeracy and ITQ level 2(or equivalent)

7. Leadership or management qualification
