Norfolk Primary Care(NPC) is an established PrimaryCare provider of clinical services. We are a key provider ofPrimary Care services in the Norfolk & Waveney system workingwith the local GP practices.
NPC has beensub-contracted to provide a home visiting service to the Practices in the NorwichPCNs. This comprisesof a team of clinicians working across the Norwich PCNs. The staff provides additional Primary Care support to that currentlybeing provided by individual practices.
Supported by a Service and Clinical Lead, the HVS team are responsible forworking with all HVS participating Practices focusing on proactivesupport to patients residing at home(including care homes).
The aim of the HVS is to reduce the number of avoidable admissions into hospital and to delivermore consistent, enhanced services from Primary Care. It offers proactive health caresupport to housebound patients, alongside the general population of patientswho may require home visiting services
Main duties of the job
This exciting role will provide a high qualityservice to housebound patients on behalf of their registered GP Practice, andthe wider PCN.
You willbe an experienced clinician who will be acting within their professional boundariesand competencies. You will provide carefor the presenting patient from initialhistory taking, clinical assessment, diagnosis, treatment, referral (where required), evaluation, and follow up ofcare.
You will demonstrate safe, clinical decision-making and expert care, including assessment and diagnostic skills,for the patients withinPrimary Care. The post holder will demonstrate critical thinking in theclinical decision-making process. They will work collaboratively
with the GP Practices to meet the needsof patients in supporting the delivery of policy and procedures, and providing nurseleadership if / as required.
In order to work at this level,those registered with the NMC / HCPC,requirements for Advanced Practice must be met.

Job Summary

Key Responsibilities:
1. Clinically examine and assesspatient needs from a physiological and psychological perspective and plan clinicalcare accordingly.
2. Assess, diagnosis, plan,implement and evaluate interventions/treatments for patients with complex needs. Proactively identify,diagnose and manage treatmentplans for patients at risk of developing a long-term condition(as appropriate).
3. Refer patients for investigation or specialist opinion, where appropriate.
4. Diagnose and manage both acute and chronic conditions, integrating both drug and non-drug based treatment methods into a management plan.
5. Work with patients in order to supportcompliance with and adherence to prescribed treatments.
6. Ensure provision of continuity of care including follow up appropriately.
7. Take guidance and instructionfrom the registered GP, when and if required.
8. Where thepost holder is an independent prescriber: to prescribe safe, effective andappropriate medication as defined by current legislative framework.
9. Provide information and adviceon prescribed or over-the-countermedication on medication regimes, side-effects and interactions.
10. Prioritise health problems and intervene appropriately to assistthe patient in complex, urgent or emergencysituations, including initiation of effective emergency care.
11. Refer patients to other health care professionals as appropriate for clinical needsand in accordance with local framework/guidelines if indicated.
12. Meet the needs of patientspresenting for opportunistic long term condition management or assessment
13. Beable to undertake medication reviews forindividuals on any type ofmedications within an agreed skill level.
