There is clear evidence that those experiencing severe mental health issues are at significant risk of dying prematurely for a number of reasons including inequalities of access to both health and social care as well as genetic and treatment-related issues.
Therefore regular physical health support and monitoring is essential to level up such inequalities and promote longevity and quality of life for those we serve.
If you choose to take this opportunity to work with us, you will be joining during a period of unprecedented transformation where our services and the Mental Health system as a whole are being redesigned via a process of coproduction with those who currently utilise our services and so know exactly what needs attention and how we might support people in the most meaningful, respectful and effective way.
This is a community-based role that requires frequent travel to various locations throughout the county including areas that may not be accessible by public transportation.
Key Responsibilities:
1. Be a visible, accessible, and professional authoritative presence for patients and staff, working alongside teams in direct clinical contact with patients and their families within the community to ensure high standards of physical health care are being carried out.
2. Ensure provision of holistic nursing assessment and high quality, safe, evidence-based care to patients.
3. Assess, diagnose, care plan, and review all aspects of physical care while supporting mental health nurses and support workers in the community to also do this, upskilling staff when needed.
4. Support the team manager in making strategic improvements to the health and safety of patients through CQUINs and institute of health improvement projects.
5. Be encouraged and supported to make improvements to care that you identify on a day-to-day basis.
6. Help carry out audits to ensure that practice is safe and analyze data to formulate strategies to improve practice within the community teams with the staff.
7. Provide cross cover to help cover leave and support workload commitments at each site; home visits will be made to some patients, and therefore a car driver is essential for this post (mileage will be paid or use of base cars).
