Rehabilitation Assistant 37.5 hours, full time or part time hours
We have one vacant Rehab Assistant post to work with Intermediate Care Service patients and provide them with the highest quality care (with a rehabilitation and therapy focus) in order to support them to maximise their recovery potential following stay in acute and community hospitals. The ICS team also supports with admission prevention.
The role of the Intermediate Care Service is to provide specialised rehabilitation to patients within interim 24hr bedded settings, facilitating smooth transfer from Acute and Community Hospital settings. To assist patients to achieve their optimal level of independence and integrate back into their communities with the appropriate level of support for them and their carers. To prevent inappropriate hospital admissions and promote a supported discharge from hospital as early as is appropriate and safe. To provide high quality multi-disciplinary (MDT) assessment and treatment, advice/support and / or referral onto other agencies.
Base/location: Wigton hospital covering the following locations: Wigton, Maryport, Carlisle and Aspatria.
Main duties of the job
1. To be able to work unaccompanied as a lone worker, without direct supervision at times but under the professional guidance and supervision of qualified staff.
2. To contribute to the assessment, implementation, evaluation and modification of team interventions.
3. Use skills to carry out simple assessments and prescribe routine equipment within the scope of the job and to a level of assessed competence.
4. To maintain accurate written and computer held records.
5. Be responsible for daily administration tasks and duties.
6. To work flexibly to meet the needs of the service and the patients which will include bank holidays and weekend working.
The essential criteria for this role includes the requirement to have a care certificate or the willingness and ability to undertake this.

Person Specification
Qualifications
* oNVQ Level 3 or equivalent qualification in health/Rehabilitation
* Care Certificate or the willingness and ability to undertake (applicable for all non-registered support workers that provide health care)
* Evidence of ongoing training and mandatory courses.
Experience
* Evidence of working within rehabilitation.
* Able to work with a wide range of people
* Experience of working in a community setting.
* Experience of lone working
* Previous experience of working in a multi-disciplinary team
Knowledge
* Knowledge and understanding of how community services generally and rehabilitation in particular support and maintain independence
* Knowledge of personal care, function and mobility, nutrition, privacy and dignity
* An understanding of how to support, with empathy and encouragement, individuals who may be unwell or have reduced functional capabilities.
* Knowledge of aids and equipment used to promote independence
* Knowledge of moving and handling policies and procedures
* Understanding of reflective practice and its application

Salary: £24,071 to £25,674 a year, pro rata
