Stockport NHS Foundation Trust is currently offering an exciting opportunity for an ACP to join our community-based Urgent Community Response (UCR) and Virtual Ward (VW) Team(s). The multi-disciplinary (MDT) team consists of GP's, Advanced Clinical Practitioners, Nurses, Occupational and Physio Therapists, Therapy Assistants and Senior Support Workers. UCR and VW work collaboratively to ensure patients receive safe, effective treatment at home, preventing the need for hospital admission. With support from our MDT, you will complete medical assessments in patient's homes to ensure appropriate intervention and treatment plans are in place to improve and stabilise their condition. As a confident communicator with strong organisational skills and a passion for continuous development, you will be instrumental in supporting and improving a high-quality service. This will include development of new care pathways and involvement in a variety of projects where you will receive regular supervision, in-service training and support with the guidance of our ACP Service Lead and Nurse Consultant.
