Main Duties of the Job
1. Undertake a range of delegated clinical care duties to deliver agreed plans of care under the supervision of trained staff.
2. Provide excellent customer care skills to ensure patients come first.
3. Ensure patients receive high quality clinical care and a good patient experience, having regard for their customs, religious beliefs, and doctrines.
4. Recognise and avoid situations that may be detrimental to the health and wellbeing of the individuals.
5. Act as an advocate for patients within the department, ensuring a patient-oriented approach to the delivery of care.
6. Provide support and assistance to professionals in the implementation of plans of care for individual patients.

Job Responsibilities
1. The post holder will be caring and compassionate and have a good nature to work within our frailty wards, be able to work with patients living with dementia and delirium, and be willing to provide support by ensuring that our service users are safe whilst promoting their dignity.
2. As directed, to record routine observations of blood pressure, temperature, pulse, respiratory rate, and blood glucose when needed. Report any abnormalities and concerns to a registered nurse.
3. Recognise when patients are in pain and discomfort and report to the nurse in charge.
4. Provide reassurance and support to patients, carers, and family.
5. Ensure own actions help to maintain quality.
6. Respond to patient requests for non-clinical information to improve communication.
7. Establish and maintain communication with people on routine and operational matters.
8. Act always in a manner which promotes positive working relationships and maintains the professional image of the Trust.
9. Under supervision, the post holder is responsible for carrying out assigned tasks as part of the clinical team in support of registered nurses, and to undertake some aspects of care provision under supervision. Ensuring the safety and wellbeing of service users, always working in a manner that promotes dignity and human rights, through the adoption of person-centred care principles.
Person Specification
Qualifications
* Evidence of a general education e.g. GCSEs or equivalent.
* NVQ II in Healthcare / QCF Diploma in Health Level 2 or equivalent qualification in the area of specialty (e.g. Operating Theatre Practice for posts in theatre) or an equivalent level of working experience within a similar role.
Knowledge
* Knowledge and experience of taking clinical observations.
* Knowledge of hospital systems.
Experience
* Some experience of working as a healthcare assistant in a hospital or registered care setting.
* Customer care experience.
