In conjunction with the Senior Clinical Nurse Stroke team and Stroke Medical Team:
1. Respond immediately to acute stroke emergencies within the ED and across all inpatient ward areas.
2. Assist in the assessment of stroke emergency cases as to the suitability for thrombolysis/thrombectomy and commence agreed treatment plans.
3. Provide a skilled clinical response involving cannulation, venepuncture, ECGs, observations and medication delivery.
4. Safely navigate and monitor all acute stroke emergencies through the agreed pathway in a timely manner, ensuring all necessary investigations are processed and reviewed, for example transfers to CT/IR/Doppler/MRI.
5. Provide direct specialist nursing care for all stroke patients, including care, information and support for the patients and their next of kin.
6. To work across the pathway by supporting the Acute Stroke Unit as a nurse leader whilst covering the emergency stroke bleep to support the Senior CNS and Medics.
7. Role model competent, skilled stroke care and pathway proven knowledge across UHPT for all patients, their families and our colleagues.
Provide clinical expertise in stroke nursing medicine over a 24/7 target time.
Work within and maintain the stroke pathway acknowledging the national stroke guidelines.
Be an integral part of the MDT.
Provide a positive, holistic and expert service.
