Role: Locum Pharmacist Banding: Band 7 Location: London Rate: £30 £37 per hour Working Hours : Full time Start Date: 13 January 2025 Duration: 3 months We are seeking a highly experienced Pharmacist to join our dynamic team at Pulse AHP. The ideal candidate will have significant experience in covering medical wards within acute NHS hospitals. Key responsibilities include validating To Take Away (TTA) prescriptions, undertaking medicines reconciliation, and providing clinical advice to the Multidisciplinary Team (MDT). Duties and Responsibilities: The pharmacist will ensure the accuracy and appropriateness of medications prescribed for patients to take home by validating To Take Away (TTA) prescriptions. They will review and verify patients' medication lists to ensure consistency and safety across transitions of care through medicines reconciliation. The pharmacist will provide expert clinical advice to the Multidisciplinary Team (MDT) on medication management and therapy. They will monitor and evaluate patients' medication therapies, making necessary adjustments to optimize outcomes. Additionally, the pharmacist will educate patients and healthcare staff on medication use, side effects, and interactions. They will ensure compliance with all relevant regulations and guidelines in the handling and dispensing of medications. Finally, the pharmacist will maintain accurate and up-to-date records of all pharmaceutical activities and patient interactions. Requirements: Essential: Up to date GPHC registration. Relevant Pharmacy qualification. Experience in a clinical pharmacy setting. Desirable: Strong communication and interpersonal skills.
