We are currently seeking a compassionate and dedicated Care Coordinator to help coordinate care for complex patients, working closely with clinical teams, patients, families, and local care systems to provide holistic, patient-centred support. Reporting to the Lead Care Coordinator, you'll thrive in a dynamic, supportive environment.
Key Details
Location: Deptford Surgery (SE14)
Hours: Full-time (37.5 hours/week), part time is considered
Salary: Up to £27,000 per year, based on experience
Key Responsibilities
* Coordinate patient care and act as the main contact for patients.
* Care and support planning.
* Schedule, reschedule, and manage appointments.
* Maintain accurate patient records and collaborate with healthcare teams.
* Support patients with chronic conditions and provide advocacy and guidance.
* Ensure compliance with regulations and contribute to reporting.
The right candidate will need to demonstrate they are:
* Organised: Able to manage tasks effectively.
* A Clear Communicator: Confident and adaptable in working with all levels.
* Analytical & Detail-Oriented: Capable of managing complex situations.
* Independent & Responsible: Able to take ownership of tasks.
* Empathetic & Compassionate: Passionate about delivering high-quality care.
* Familiar with medical systems (e.g., EMIS, SystmOne).
* Have strong computer skills and understanding of medical terminology.
* Knowledge of local health & social care systems.
* Ideally, experienced in care coordination or familiarity with EMIS and eRS (not essential).
