Person specification Criteria Essential Desirable Qualifications Recognised degree in adult nursing Registered Nurse on Part 1 of the Nursing Midwifery Council register Evidence of further study Mentorship or training qualification District Nursing qualification / community practitioner pathway or willingness to commence within 1 year of starting Nurse prescriber or willingness to undertake training Relevant post-graduate qualification, e.g. in elderly care, chronic disease management, long term conditions, rehabilitation Experience Post graduate accumulative experience in the fields of elderly/ rehabilitation/ community/ intermediate care/ long term conditions/acute illness. Experience of delivering education and teaching. Proven and evidence of experience in an autonomous practitioner role Experience of working in the community in healthcare.
