LDA Hospital at Home is an innovative service being developed under the Thames Valley Provider Collaborative, focusing on young people with moderate to severe learning disabilities and/or autism-related functional impairments. The service aims to support young people at risk of home or placement breakdown, preventing unnecessary admissions and stepping down from inpatient care.
By utilising new technology and providing care closer to home, the service reduces admissions, length of stay, and waiting times, offering earlier discharges from inpatient care. We aim to minimise the burden on families, enhance engagement with community teams, encourage early therapeutic home leave, and ensure the best care pathway for each young person.
The role is based at the Meadow Unit at our Warneford Site in Oxford. We offer flexibility in hours and welcome applications for both part-time and full-time positions.
Access to transport for home visits and community visits is essential. If necessary adjustments can be considered in line with the Equality Act 2010.

As the CAMHS community nurse in the team, you will work as part of the multi-disciplinary team to assess, plan, implement and evaluate the specialist health care needs of children and young people who have a learning disability and/or Autism.
The responsibilities of the role include, but are not limited to:
* Provide quality nursing care including clear risk assessments in accordance with the individualized child or young person’s Care Plan and to work together with a senior nurse and the rest of the MDT in the monitoring of that care.
* To work in partnership with others to identify and assess the nature, location, and seriousness of the particular risks.
* To ensure that all clinical and risk assessments are completed and reviewed in line with Trust policies.
* To work with other members of the multi-agency team to plan, monitor and review the effectiveness of the risk management plan.
* Contribute to effective multidisciplinary working within the Team and build effective liaison with other health and social care agencies.
