We are looking for a dedicated and experienced Client Liaison Officer to join our management team and help maintain these high standards. Position Purpose: The Client Liaison Officer is a key role within our team, ensuring that care services are provided in line with individual care plans, service agreements, and the required standards. This is a hands-on role, involving the supervision of care teams, assessing client needs, and co-ordinating the delivery of care services. The successful candidate will be responsible for liaising between clients, staff, and management to ensure the seamless provision of care. Key Responsibilities: As a Client Liaison Officer, you will: Conduct needs assessments and develop individual care plans for clients. Ensure continuity of care by managing and implementing an organised rota system. Supervise care workers and teams, offering guidance, advice, and mentoring as required. Act as a point of contact for care workers seeking advice and support with any challenges. Provide direct care in emergency situations or out of hours when necessary. Conduct spot checks on care delivery and report findings to management. Regularly visit service users to gather feedback and address any concerns or issues. Investigate and report on concerns, including safeguarding issues and complaints, to the service manager. Assist with staff induction and training, and assess new staff for their ability to work independently. Organise care reviews and ensure that any changes to care plans are implemented. Review daily notes, incidents, and concerns to ensure high-quality care. Provide support to the senior management team, including Registered Managers (RM). Keep up to date with company policies, procedures, and relevant industry guidelines. Collaborate with other professionals involved in the care of clients to ensure comprehensive support. Contribute to the service’s quality improvement and business planning strategies. Engage in continuous professional development through relevant training and qualifications. Represent New Age Care in a professional manner when liaising with external agencies. Work flexibly as part of the team to meet service requirements. Ensure compliance with the Health and Care Act 2012, and CQC 5 Domains: Safe, Caring, Effective, Responsive, and Well-led. Follow national guidelines such as NICE guidelines and Skills for Care. Undertake any administrative or ad-hoc duties as directed by your manager. Key Requirements: Previous experience in a care coordination or related care role is essential. Relevant qualification (Certificate/Diploma) at an appropriate level. Strong leadership and supervisory skills. Excellent communication, interpersonal, and organisational skills. Ability to work flexibly and respond effectively to emergencies. Commitment to continuous professional development. Knowledge of the Health and Care Act 2012, CQC standards, and national guidelines.
