Job summary The post holder will be required to deliver and lead the Integrated Community Stroke Service (ICSS) providing highly specialist assessment and treatment for adults with stroke across Cornwall. The Team Lead will ensure all patients with stroke with rehabilitation needs have access to specialist assessment and associated care. Additionally, they will support the Consultant Practitioner for Stroke and the ICSS Manager. They will operationally manage the Locality Team and act as a role model and expert clinical practitioner. The Team Lead will provide leadership for the specialist stroke team through supervision, training and appraisal alongside professional clinical supervision. They will also lead on clinical governance and service development alongside the Consultant Practitioner in Stroke and in partnership with other clinical leads. This is a fixed term post for 12 months. The post is a community based role and applicants must have access to a vehicle which can be used for work purposes. Please note not all office locations or client care bases maybe reached by public transport. Main duties of the job Support the Consultant Practitioner for Stroke and the ICSS Manager. They will operationally manage the Locality Team and act as a role model and expert clinical practitioner. Use expert and advanced clinical reasoning to formulate and deliver an individual Occupational Therapy treatment programme based on a sound knowledge of evidence-based practice involving the patient, carer and/or appropriate staff.
