Responsibilities:
* Collaborate and develop IAPs (Individualized Action Plans) with consumers and other staff when appropriate
* Assist with community integration
* Provide medication administration
* Support activities in the community
* Develop short term goals with consumers to assist with integration into the community
Requirements:
* Bachelor's degree in either Psychology or Social Work or a related field
* Valid driver's license
* Reliable vehicle
* Ideally 2-3 years prior experience working with adults having severe and persistent mental illness
Schedule: Sunday-Thursday, 3:00pm-11:00pm (40 hours)
