Job summary
We are looking for an enthusiastic Clinical Pharmacist to join our well organised GMS practice.
We are a very friendly practice with a really good team spirit.
Working hours can be flexible and involves Hybrid working but to include some time in the practice.
Main duties of the job
The Clinical Pharmacist will support the Partners and otherclinical staff in the surgery to deliver a programme of medicines optimisationand review, including managing hospital discharge medication changes, secondarycare implemented medicines, CQC related medicine changes and practicemedication queries. The post holder willbe responsible for encouraging the development of better understanding of theprinciples of medicines optimisation for the clinical staff within thePractice.

2 Male Partners, 3 Female Salaried GP's, 1 Male Paramedic and 1 Female Paramedic both working 8 sessions/week.

Qualifications

17. - Degree in Pharmacy
18. - Registration with General Pharmaceutical Council
19. -Completed or willing to commit to completing CPPE pathway or other primary care training pathway
