Job overview
The family health practitioner will work as part of a skill mix team delivering the healthy child programme to school aged children in the Wakefield District. The post holder will work as part of the team addressing health inequalities and public health priorities within the area.
Main duties of the job
Supporting the 5-19 Family Health Team in the delivery of targeted interventions using a tiered approach (universal, universal plus and universal partnership plus).
Working with other agencies to ensure a co-ordinated approach to addressing health inequalities.
Contribute to the development, delivery and reporting of health promoting activities within a variety of settings.
Deliver evidence based early intervention programmes to support children, young people and families e.g. Speech and language development, prevention of accidents in the home, promotion of oral health, promotion of healthy weight and physical activity.
Maintain contemporaneous and accurate records as required by HDFT and in line with professional standards.
