The post holder will be responsible for undertaking Holistic assessment for services users over the age of 55 across Knowsley who require assistance to maintain independence and improve safety within the home. They provide support to service users who may have concerns regarding finances and pensions.Assessments are completed in service users homes and across the community. They will be required to assess the environmental and physical health needs of individual. to identify, implement and review low level adaptations or short-term interventions.Post holder will Identify any appropriate equipment and demonstrate its use. The Post holder will refer to partnership agencies where required, to assist with implementation of low-level adaptations and home safety equipment.The postholder will deliver identified health promotion advice/ activities to individual and groups of older people to increase their knowledge of skills in relation to health and wellbeing to make better health choices.The post holder will be required to lead Chair based exercise groups within the community setting. This will involve leading exercise sessions to a small group of patients, completing risk assessments of venues, and physical activity readiness risk assessment forms for all service users. Identify and implement new groups as the service requires.The post holder will work as part of a team that works closely and supports a wide range of other services to promote health and wellbeing across the whole Community including supported Living facilities.The post holder will build positive relationships with both internal NHS professional services and external partners such as Knowsley Metropolitan Borough Council, Merseyside Fire & Rescue Service, Health watch, Better Lives and Care & Repair. They provide support to service users who may have concerns regarding finances and pensions.The Post holder will be expected to use shared decision-making techniques to agree and facilitate an individual based plan which best suits the patient, to improve safety in their environment. The Post holder will support service users in maintaining maximum level of mobility and independence and reducing their risk of falls. This may include identification of suitable indoor walking aids.The Post holder will contribute to the learning and development of new team members and students. Assisting with identification of learning objectives, monitoring progress and providing support through managerial and clinical supervision.
