Recently awarded XPERT Organisation of the Year for 2024, we are now looking for a highly motivated and enthusiastic person to join our dynamic team as a Diabetes Educator.
The role is to deliver the XPERT Diabetes and Diabetes & You programmes to patients with type 2 diabetes in group settings, both in person and using an online platform. You will need to be passionate about supporting people to manage their diabetes, have excellent communication skills and confident in working with people in a group environment. You must be willing to travel across Derby & Derbyshire.
The successful applicant will be fully supported and undertake full training to deliver the education programmes.
You will deliver education sessions for people diagnosed with Type 2 diabetes in a group setting, demonstrating expert knowledge and skilful group management, following the course curriculum.
You would be required to educatepeople with diabetes to understand their health status and needs, and support them to work towards behaviour change with best practice advice either in a group setting or via the telephone/e-mail or social media sites.
You would be required to ensure that on completion of the course patients, carers and relatives are equipped with the necessary skills and education to actively manage their own condition, improving quality of life.
You will be expected to participate in the Performance and Development Review (PDR), including the production of Personal Development Plans (PDP) and take responsibility for achieving agreed objectives.
You will be responsible for maintaining contemporaneous and accurate patient records and documentation in line with DCHS policies and procedures.
You will be expected to use specialist knowledge of Diabetes to provide expert advice and deliver group education sessions, guided by policies (e.g. NSF, NICE).
You will be required to use your own initiative to manage the different learning needs of individuals within the group setting.
You will be based in suitable locations to deliver education sessions (e.g. health centres, leisure centres and community rooms) and there may be requirements to work some evenings & Saturday's.
