We have an exciting opportunity for an enthusiastic band 6 physiotherapist to join the work of the multidisciplinary team at the Day Hospital. Applicants must be passionate about rehabilitation and support of independence in the older person.
The Day Hospital is an award winning service, with Practice Development Unit Accreditation. We use an extended scope of practise framework for holistic healthcare management. The Multidisciplinary Service specialises in management of falls, stroke rehabilitation, and Parkinson’s disease as well as health promotion strategies and treats a wide range of multi pathologies affecting the older person.
We run a number of exercise and education groups and provide admission avoidance assessment and intervention in patients own homes. Our outreach team also visits patients in their own homes following discharge from the acute hospital to ensure a safe discharge by managing any immediate risks such as equipment needs and falls home hazard assessments. The post holder will expected to work confidently across all these streams of work.
A full driving licence is essential for this post.
