We are seeking a Specialist Doctor to join our team toprovide high-quality care within our inpatient units and as required, the Hospital at Home service across Bath and North East Somerset! This is a great opportunity for an established clinical leader looking for the next step in their career or a change in location to be part of a fantastic service which continues to evolve.
In this role, youll deliver medical care primarily to older, frail patients with complex medical needs, helping to bridge the gap between hospital and community-based services. Working alongside multidisciplinary teams, youll play a vital role in supporting patients transitioning from acute hospital care to our rehabilitation and stroke beds.
Primarily based on theinpatient unit at St martin's Hospital but able to cover Paulton Hospital when required, youll focus on delivering high-quality care within these settings. Looking ahead, there are plans to integrate support for the Hospital at Home service, extending care into patients' homes.
The position is full-time; however, candidates seeking reduced hours are also encouraged to apply and will be considered. The role is primarily Monday to Friday; however, flexibility across 7 days and potentially outside of core hours may be expected, with flexible start and finish times
Main duties of the job
As aSpecialist Doctor, you will:
1. Provide medical input and take clinical responsibility for patient care in community hospital inpatient units. Plans for role to include future rotational cover for our Hospital at Home service.
2. Examine new admissions promptly, document differential diagnoses, and create management plans in the MDT patient record.
3. Collaborate with advanced clinical practitioners, consultants, and the broader MDT; participate in weekly MDT meetings and ward rounds.
4. Refer patients to other specialties or clinics in consultation as appropriate.
5. Initiate and document advance care planning discussions.
6. Manage acute issues during patient rehabilitation.
7. Liaise with out-of-hours services as needed.
8. Work with the MDT on discharge planning, ensuring safe, effective care.

We are looking fora Specialist Doctor who has:
Full GMC Registration (or eligibility), with a current license to practice. Relevant qualifications such as MRCP, Diploma in Geriatric Medicine, MRCGP, or an advanced degree (MD, PhD, MSc). Minimum of 4 years postgraduate experience, with a background in assessing and treating elderly patients in inpatient or community settings. Experience working within a multidisciplinary team (MDT). Proven commitment to continuing professional development (CPD), with involvement in audits and service improvement projects. Skilled in critically appraising new treatments and developments. Confident in managing clinical risk within community care environments.
