Job summary We are seeking a dedicated and compassionate Band 5 Physiotherapist to join our Childrens Community Health Team in Surrey. As a Band 5 Physiotherapist, you will: Provide professional, specialist assessments and treatment to children referred to our physiotherapy service. Empower children and their families to achieve maximum independence and self-management in daily activities. Collaborate with colleagues across health, social care, education, mental health, and the voluntary sector to ensure a holistic and joined-up approach to care. Support carers and assistants in managing risk and guiding families to appropriate community and voluntary resources. Lead in the assessment, diagnosis, and creation of tailored physiotherapy treatment plans. Act as the physiotherapy lead in multi-disciplinary team settings, offering expert advice to health professionals, education services, social services, and voluntary agencies. Ensure smooth care pathways through effective communication and liaison with other services and agencies. Base: You will be based at one of our Surrey-wide office hubs, with the flexibility of hybrid working to support your professional and personal needs. Main duties of the job Support and Training: Provide support, advice, and training to parents, carers, clients, and other professionals. Oversee and delegate tasks to assistants, students, and volunteers within the therapy team. Patient Care: Develop and implement individualised care plans, promoting self-management and independence. Act as a key worker to coordinate care across multidisciplinary teams and external agencies. Communication and Collaboration: Communicate effectively with patients, families, and professionals, overcoming barriers using techniques such as Makaton or communication aids. Liaise with health, social care, education, and voluntary sectors to ensure joined-up care delivery. Specialised Responsibilities: Assess, recommend, and coordinate the provision and safe use of specialised equipment for children and families. Provide advice and motivation to patients/carers with physical or psychological barriers to engagement.
