The stoma and colorectal team here at Mid Yorkshire teaching NHS trust is excited to announce that we have an opportunity for a band 6 Stoma clinical nurse specialist to join our team, we are a supportive team who provide care and management to patients within the acute care setting.
We are looking for a highly motivated, focused nurse who will be committed to the teams vision of providing evidence based high quality specialist ostomy care. You will part of a wider MDT who work in collaboration to achieve a good pathway for patients in our care
Main duties of the job
1- To gain extensive knowledge in Bowel cancer, irritable bowel disease, and bladder cancer
2- To carry out preoperative counselling and the siting of stomas for patients prior to surgery
3- To manage and care for stomas, including complex stomas / fistulas and wounds
4- To remove bridges from stomas
5- To manage and advice staff including the medical team, patients and carers / family on recognition and management of high output stomas
6- To manage and care for mitrofanoffs and ileal conduits
7- To remove Stents from Ileal conduits
8- To facilitate and promote the care of post operative patients including promoting the enhanced recovery programme (eras)
9- To provide education on stomas, the care and management of them to staff, partake in trust training to wider staffing establishment.
10- To provide education / training of care and management of stomas to patients, their families and carers.
11- To work as an autonomous practitioner carrying out stoma review clinics, 6-week reviews and end of treatment clinics
12- To work collaboratively with the wider MDT and community stoma team including the stoma sponsorship team
13- To attend stoma wellbeing events and community stoma support groups.

Will have continuous delegated responsibility for a caseload of patients and will provide an effective and patient focused service which is considerate on needs of users and providers.
Organises, prioritises and evaluates workload based on accepted referral criteria and discharge criteria in line with operational protocols.

Work within national policy frameworks relevant to the service and trust local guidance.
Review, interpret and act upon diagnostic investigations and point of care testing which may effect, change to individual care plans, liaising with appropriate medical staff where necessary.
Deal effectively and professionally with patient / service user issues collaborating with PALS as appropriate and take a proactive approach to resolving issues and learning from complaints and escalate when unable to resolve.
To maintain accurate appropriate and concise patient records within agreed databases and electronic systems.
Contribute to the multi-professional approach in the management of patients / carers within the specialist area in relation to ongoing care needs.
Act as a resource for specialist knowledge for clinical staff on wards / departments.
Ensure effective multi- disciplinary communication.
Support the management function of the department by provision of reports and information as required.
Provide mentorship and preceptorship as required to junior staff in the team or students on placement.
Ensure you work within an agreed divisional work plan.
