Wyre Integrated Network PCN are recruiting for a Care Home Administration Assistant to join their team. The successful candidate will join a team consisting of Pharmacists, Paramedics, Social Prescribers, Care Coordinators, GP Assistants, and a Physiotherapist supported by a team of GPs and clinical leads all striving to support the needs of the local community.
The PCN Administration Assistant is responsible for undertaking a wide range of administrative duties and the provision of administrative support to the PCN Care Homes and Social Prescribing Team.
The Administration Assistant will assist the Social Prescriber with their relationships and engagement with the PCN practices, Care Homes, and health care professionals within the community, enhancing continuity of care.
The Administration Assistant will also be responsible for undertaking a wide range of administrative duties in support of the multidisciplinary team, including overseeing the coordination of the Care Home MDT meetings and facilitating weekly care home rounds by identifying residents requiring review or discussion. They will work closely with the Care Home Leads and primary care professionals within the Primary Care Network (PCN) to support the management of a caseload of care homes under our care, ensuring effective communication between Primary Care and care home staff.
Main duties of the job
As our Care Home Administrator, you will:
* Proactively identify and collaborate with the care homes social prescriber to support with the administration of personalised care support plans for residents by the care home team.
* Assist the care home team to support residents and families in managing their healthcare needs, answering queries from care homes, and facilitating referrals to the Care Home MDT or escalating concerns to the Clinical Team.
* Coordinate and direct health and care services for residents and their carers, working alongside social prescribing link workers, health and wellbeing coaches, and other primary care professionals.
* Support the organisation with the delivery of Care Home MDTs within the PCN.
* Build and maintain effective relationships with GPs and primary care professionals to enhance the coordination of care for the residents.

Job responsibilities
You will guide the coordination of Care Home MDTs and facilitate weekly rounds with the GP practices ensuring that residents in need of a review or a discussion are identified and prioritised. You will collate information to help individuals prepare for clinical discussions with primary care professionals. Working closely with Care Home Leads and other primary care professionals within the PCN, you will complete the administration for a caseload of care homes, ensuring timely assistance for staff and residents, and addressing changing needs. You will also support the implementation of local priorities, address health inequalities, and promote population health. This role is designed to improve communication between Primary Care and care home staff, aiming to prevent unnecessary GP visits and hospital admissions. The post holder must be adjustable, as the role will evolve alongside the Enhanced Health in Care Homes Framework initiative. The role will involve providing administration support for the Social Prescribing Team, supporting education of non-clinical and clinical staff within their PCN multi-disciplinary teams on what other services are available within the community and how and when patients can access them.
