PROGRAM PURPOSE:
The National Association of Chronic Disease Directors (NACDD) is a national public health non-profit agency focused on improving the health of the public by strengthening state and national leadership and expertise for chronic disease prevention and control. NACDD promotes health and reduces the burden of chronic disease through various programs and initiatives in collaboration with the CDC.
To enhance public health resources across the country and support state and local public health settings respond to and recover from the COVID-19 pandemic, the Public Health AmeriCorps (PHA) program supports the recruitment, training, and development of the next generation of public health leaders.
SPECIFIC SERVICE FUNCTIONS AND RESPONSIBILITIES:
* Support the St. John United Methodist Church in activities for the Nanticoke Lenni Lenape Native American community.
* Support nutrition and food security services as well as clothing distribution services.
* Participate in hands-on services such as food access and distribution.
* Conduct outreach to the targeted community, including health care providers.
* Organize and distribute food to over 300 recipient families.
* Track and monitor records required by the US Department of Agriculture for each enrolled household.
* Establish a computerized client enrollment and participation system (OASIS).
* Assist in developing a mobile produce stand to visit local neighborhoods.
DESCRIPTION OF ANTICIPATED SERVICE SCHEDULE:
* Hybrid, usual service hours will be 80% onsite and 20% virtual.
* Service hours are to be completed Monday-Friday between 8:00am and 5:00pm ET.
* A maximum of 20% of service hours may be training, education, or other approved activities.
* A maximum of 10% of service hours may be fundraising activities.
DESIRED SKILLS & COMPETENCIES:
* Proficiency with computerized recordkeeping systems including WORD, EXCEL, and Microsoft platforms.
* Experience with a culturally diverse rural environment.
* Willing and able to travel within the community.
* Health and nutrition education experience preferred; Bilingual Spanish-English a plus.

REQUIRED COMMITMENT:
Service members are expected to complete 1200 service hours within a one (1) year timeframe, beginning 02/03/2025 and ending 02/02/2026.
ORIENTATION AND TRAINING:
* Pre-service orientation
* PHA-specific training
* Program-specific training
* Any other trainings deemed necessary by the Host Service Site supervisor
EVALUATION AND REPORTING:
* Complete quarterly assessments of the PHA Program.
* Complete monthly reflection forms.
* Participate in quarterly performance evaluations with their Host Site supervisor.
* Submit service timesheets biweekly.
PUBLIC HEALTH AMERICORPS ELIGIBILITY AND REQUIREMENTS:
* Be at least 18 years of age.
* Have a high school diploma or its equivalent.
* Be a citizen, national, or lawful permanent resident alien of the United States.
* Satisfy the National Service Criminal History Check eligibility criteria.
* Live within commutable distance of the Host Site.
OTHER PROGRAM ELIGIBILITY AND REQUIREMENTS:
Preferred: Upper-level undergraduate or graduate student pursuing a public health degree or related field.
Required: Compliance with all Host Service Site vaccination requirements, including for COVID-19.
