Structured Clinical Management (SCM) Practitioner

This is a development opportunity for an experienced Mental Health Nurse/ Social Worker/ Occupational Therapist to join Carshalton and Wallington Integrated Recovery Hub. You will be delivering Care Coordination and supporting the IRH as part of your role, with the other part of the role supporting Structured Clinical Management (SCM) within the team. SCM is an evidence-based approach for working in a community setting with people with Complex Emotional Needs/ a diagnosis of personality disorder. You will receive training and ongoing supervision to ensure that you develop and maintain the necessary skills for working with this client group, including assessments for Complex Emotional Needs (CEN; difficulties consistent with the diagnosis of personality disorder), delivery of the Structured Clinical Interview for Diagnosis of Personality Disorder (SCID), internal SCM introductory training including running SCM groups, and further training in related therapeutic approaches. Supervision will be provided by a Clinical Psychologist.
Main duties of the job
You will have some Care Coordination duties aligned with this team. You will also undertake clinical duties under the SCM Team, where you will receive your clinical supervision. Your SCM duties will include assessments, coaching calls, running SCM groups, and consultation to other professionals. You could undertake some SCM treatment sessions if appropriate, following training. As such, this is an excellent development opportunity for those wishing to extend their experience and skills in working with this client group.
Main duties of the job
Care Coordination of a small caseload of non-SCM service users.
Attending IRH and SCM Team meetings.
Assessment of clients with a diagnosis of personality disorder under the SCM model, including SCID assessments.
Delivering treatment interventions in the group modality.
Comprehensive risk assessment and management.
Consultation to other professionals regarding specific service users.

Job responsibilities
* To implement, in conjunction with other members of the team, plans for the formal psychological treatment and/or management of a patients mental health problems using SCM.
* To exercise autonomous professional responsibility for the assessment and treatment of allocated patients within the service model.
* To participate in clinical and managerial supervision.
* To be responsible for the flexible management of a defined caseload designed to meet the individual needs of the client and carers.
* To work within policy, professional and legal frameworks at all times and have a clear understanding of ones Code of Professional Conduct, risk assessment and safeguarding children protocols, Mental Health Act and CPA, and Trust clinical policies
* To ensure effective communication within the team and share relevant information as required.
* To implement and/or participate in crisis intervention measures with the client/family or in conjunction with other agencies.
* To supervise and monitor the therapeutic effects and side-effects of prescribed medication providing the client with clear advice and information.
* To maintain accurate clinical documentation in line with Professional and Trusts record keeping standards.
Person Specification
Experience and Knowledge
* Experience of reflective practice, e.g. understanding and ability to use supervision to reflect on skills, attitude and knowledge and develop these by using evidence-based practice
* Significant relevant clinical experience
* Evidence of continuing professional development
* Knowledge of clinical governance and audit
* Knowledge of relevant professional and clinical national policy/ legislation, specifically for clients with a diagnosis of personality disorder
* Experience of Interagency working
* Experience of managing a caseload
* Experience of working in the community
* Experience of supporting practice development in clinical areas
* Experience in the delivery of psychological therapy, particularly for clients with a diagnosis of personality disorder
* Awareness/knowledge of the issues surrounding social inclusion for mental health service users
* Experience as a preceptor/ mentor
Education and Qualification
* Current professional registration (Nurse, Occupational Therapist, Social Worker, Clinical/ Counselling Psychologist) OR Clinical Associate Psychologist OR undergraduate degree plus post-graduate practitioner diploma in psycho-social interventions or a related field
* Training in SCM or other evidence-based intervention for this client group
* Post-registration qualification/ accreditation relevant to the post
