Due to Wiltshire Health and Care's drive towards a resilient and skilled workforce, we have an exciting opportunity that has arisen within the City Community Team for a qualified Community Nurse Specialist or an experienced Band 5 Community Nurse to undertake this role. The SPQ is an initiative that seeks to develop our own staff into the leaders of tomorrow, and you will be released to attend locally held action learning sets and benefit from peer support. You will be responsible for line managing junior members of staff and managing one of the caseloads within the team. This position will offer the successful candidate the opportunity to develop both professionally and personally; optimizing and enhancing skills and competence while working alongside a supportive multi-disciplinary team. The successful applicant will also be encouraged to contribute to the development and implementation of new ways of working to continually evolve care delivery for our Wiltshire patients and maximize the services within the Community Team.
You will put to use your strong communication and organizational skills, along with a flexible approach, to ensure you deliver positive experiences to patients at every stage of their healthcare journey. You'll care for patients and their families in the way that you believe is best, delivering our high service standards, sharing best practices, and actively improving the way we work to help us continue to change things for the better. Our ideal candidate will be / will have:
* Registered Nurse (RN) with current live PIN on NMC register
* Teaching and Assessing NMC approved qualification
* Evidence of relevant post-registration specialist professional development
* 3 years' experience as a registered nurse
* Experience in a variety of settings including hospital and community
* Experience of managing a range of long-term conditions
* Experience of working collaboratively with Social Services
* Evidence of innovative clinical practice development
As this role requires travel, a driving license and access to a car is required for this role.
