Hours: Monday to Friday: 08:30 - 16:30. There will be an expectation to work some weekend and night shifts, as required.
Salary: This position is full-time, you'll receive an annual salary between £41,094 - £47,466 per annum - depending on experience.

Main duties of the job
As a Planned Care Lead, your responsibilities will vary. You will:
* Lead a multi-disciplinary team to deliver a nurse-led service and prevent hospital admission.
* Work with Head of Healthcare and Deputy Head of Healthcare to develop processes and methods to treat, inform and guide patients through their treatment, recovery, and long-term goals.
* Provide visible leadership, being accountable for the quality and assurance agenda, continually reviewing care and identifying areas of learning within your team.
* Introduce and embed new ways of working through innovation and respond to new service requirements, whilst continually enhancing care provided to our patients.

Qualifications & Experience Required
* NMC Registered General Nurse or Paramedic with current HCPC registration
* Evidence of Continued Professional Development
* Significant post-registration experience in clinical primary care practice
* Experience managing teams within regulated environments
* Understanding of the complexities of multi-disciplinary and multi-agency working

Job description
This role will lead our Planned Care Team, which is a bespoke multidisciplinary team delivering routine planned, follow-up, and recovery care for patients. As a Senior Nurse, you will lead a multi-disciplinary team to deliver a nurse-led service and prevent hospital admissions. The Planned Care team is made up of a variety of registered and non-registered professionals ensuring our patients receive integrated and coordinated care.
You will provide visible leadership, being accountable for the quality and assurance agenda, continually reviewing care and identifying areas of learning within your team. Working with the other team leads, you will ensure all team members from Early Days, Planned Care, and Unplanned Care teams are well supported through regular clinical and management supervision.
Your role will include the development and implementation of care plans, monitoring and implementing care for those on the complex care register, contribution to MPCCC (multidisciplinary meeting), meetings with HMPPS colleagues, and close working with our social care patients and partners. You will also run regular clinics for patients.
