Concord Medical Practice are looking for an enthusiastic Practice Clinical Pharmacist to work closely with our team. The pharmacist will be expected to contribute to the leadership of medicines-related issues for the surgery.
Main duties of the job
The post holder will work as part of a multi-disciplinary team with the PCN pharmacists in a mainly patient-facing role. The post holder will take responsibility for areas of medicine management, clinical medication reviews, supporting the general practice staff with regards to prescription and medication queries. They will provide support for the repeat prescriptions system, deal with acute prescription requests, medicines reconciliation on transfer of care and systems for safer prescribing, providing expertise in clinical medicines advice, while providing clinical leadership on medicines optimisation and quality improvement and manage some aspects of the quality and outcomes framework and enhanced services.

We are looking for a Clinical Pharmacist to join us for 10 hours per week working remotely.
Job responsibilities
1. Working in conjunction with the PCN pharmacy team.
2. Collaborating with the practice manager to ensure that all aspects of medicines management happen smoothly across the practice.
3. Updating policies and practices of medicines management in conjunction with the prescribing lead.
4. Shared care monitoring, including maintaining searches and liaising with the hospital on all shared care medications, oral anticoagulants, DOACs, and any other high-risk medicines that require monitoring.
5. Initiating patients on DOACs.
6. Ensuring safe policies and practices of warfarin management and carrying out necessary audits for the enhanced payment schemes.
7. Ensuring all new patients have medication reviews, recalls added and repeat prescriptions sorted in time for when needed.
8. Reconciling medications and handling hospital tasks and requests on selected clinic and discharge letters.
9. Carrying out medication reviews on more complex patients and those who are frail and may require a face-to-face or telephone review.
10. Dealing with on-the-day pharmacy and patient queries from reception and clinicians.
11. Deprescribing opioids or other drugs of addiction or high-risk medication appropriately with support from the prescribing lead.
12. Monitoring patient discharges from hospital on opioid medication.
13. Contributing to & participating in research studies in the practice.
14. Contributing to learning and significant safety incidents concerning medication in the practice.
15. Monitoring MRHA alerts and other alerts that come from ICB or other sources, auditing areas of potential concern, and contributing to improvements in safety.
16. Keeping the practice updated on safety and prescribing issues and disseminating this information to all prescribers at the monthly management meetings.
17. Looking at areas of prescribing in QOF, CQC, ESDS and other LESs or DES as required to help the practice in these areas.

Essential Qualifications
* Registered pharmacist with the General Pharmaceutical Council.
* Previous experience in primary care setting.

* Knows and follows professional and organisational policies and procedures relating to the role.
* Understanding of primary care, community pharmacist and management of some medical conditions acute or long term.
