Responsibilities Assess needs - Evaluate patients' needs and goals Create care plans - Develop proactive care plans for patients Monitor care - Visit patients to ensure that the care they are receiving is satisfactory Adjust care plans - Revise care plans when there is a change to a clients health Conduct reviews - To maintain high standards in terms of service delivery Help with transitions - Assist patients with transitions between different types of care Support patients - Encourage clients to become more involved in their own care and health Skills and Qualifications Strong communication skills A positive attitude Proficient in the use of Microsoft Good organisational and prioritisation skills Ability to work well under pressure Attention to detail Ability to build relationships Relevant experience within the healthcare sector (preferred) Must hold a full UK driving license and have access to your own vehicle
