Job summary A Medicines Care coordinator helps to improve the continuity of care around medicines by acting as a point of contact for patients, families and professionals. Care coordinators provide extra time, capacity and expertise to support individuals in preparing for, or in following-up clinical conversations they have with primary care professionals. They will work closely with the practice prescription teams, PCN Pathway providers and other primary care professionals within the PCN to ensure coordination and delivery of safe and efficient care with medicines, as possible. They focus on delivery of comprehensive models of care concerning medicines to ensure safety and optimise patient outcomes to reflect local priorities, health inequalities or population health. The post holder will play a key role at the heart of our GP Practices, working alongside other PCN colleagues to deliver holistic and coordinated care to our patient community. They will work as an integrated member of the PCN Pharmacy Team. They will be responsible for the co-ordination and delivery of specific projects to ensure the achievement of key health and welfare objectives of Folkestone, Hythe and Rural PCN. Main duties of the job The post holder will work alongside PCN Pharmacy team members, clinicians and other staff over the seven practice sites and as part of a multi-disciplinary team. They will take responsibility for an area of work that will contribute to fulfilling the requirements of the Network Contract Directed Enhanced Service (PCN DES). This role is designed to ensure new pathways and/or services are co-ordinated efficiently and effectively, ensuring robust communication with all stakeholders. Working alongside Clinical Pharmacists and Pharmacy Technicians on projects to enhance medicines usages in the following settings; care homes, patients own homes and clinics. Working with the Pharmacy Technicians in the co-ordination of support to patients with concordance problems. Provide a point of contact for both staff and patients, in coordinating care and will help to deal with incoming queries relating to PCN level pathways or processes. Help people to manage their care needs around medicines through answering queries, making and managing appointments and monitoring, and ensuring that people have good quality written or verbal information relating to their care. As well as reactionary work, the role includes projects to proactively identifying care needs not being met and working as an MDT team to resolve issues. About us Our PCN has a patient population of just under 50,000 across 7 practices. The PCN team is now around 40 staff delivering 12 centralised services, we have our own office base providing an excellent working environment for staff, this office is very close to our Lead Practice. We have a very motivated work-force always striving to provide excellent care for our patient population. All staff are valued and have opportunity to share ideas for future developments. Our Clinical Director is often working from the PCN offices and has an open door policy. We have just incorporated our new Limited Company and will be submitting our CQC application soon.
