The post holder will work within their clinical competencies as part of a pharmacy team to provide expertise in clinical medicines management, provide face-to-face structured medication reviews, manage long-term conditions, management of medicines on transfer of care and systems for safer prescribing, manage repeat prescription authorisations and re-authorisation, acute prescription requests, while addressing both the public health and social care needs of the role. This position is pivotal to improving the quality of care and operational efficiencies, requiring motivation and passion to deliver an excellent service within general practice.
Main duties of the job

* Have completed or be working towards the CPPE Development Pathway
* Perform structured medication reviews (SMRs) to proactively manage patients with complex polypharmacy and care home residents
* Be involved in the management of long-term conditions
* Embrace service development to grow their capabilities and the clinical pharmacy team
The ideal candidate will be a Pharmacist with clinical experience, and being an independent prescriber is desirable. We are looking for someone who is committed to improving patient care and is passionate about clinical pharmacy. The candidate must be able to work effectively as part of the practice team and be comfortable in encouraging new ways of working. While prior experience in General Practice would be an advantage, pharmacists from other sectors with transferable skills are very welcome.

* Help develop our reputation for training with projects such as providing Pre-registration Pharmacy Placements
* Shape the future direction of Clinical Pharmacy in primary care
The ideal candidate will be a Pharmacist with significant clinical and quality improvement experience and be an Independent Prescriber. We are looking for an individual with leadership qualities, who is committed to improving patient care and is passionate about clinical pharmacy in Primary Care. The candidate must be able to work effectively as part of the practice team and possess the skills and resilience to manage rapid change. Although prior experience in General Practice would be an advantage, pharmacists from other sectors with significant transferable skills will be considered for the role.
Qualifications
Essential
* GPhC registered pharmacist and must hold a GPhC independent prescribing qualification.
* Have a minimum of 2 years working as a pharmacist demonstrated within a practice portfolio.
* Experience in managing pharmacy services in primary care.
* Experience and an awareness of common acute and chronic conditions that are likely to be seen in general practice.
* Ability to communicate complex and sensitive information effectively with people at all levels by telephone, email, and face-to-face.
Desirable
* Minor ailments certification, membership of the Royal Pharmaceutical Society.
* Working towards faculty membership of the Royal Pharmaceutical Society
* Medicines Management Qualification
* MUR and repeat dispensing certification
Experience
Essential
* Minimum of two years working as a pharmacist demonstrated within a practice portfolio.
* An appreciation of the nature of GPs and general practice. An appreciation of the nature of primary care prescribing, concepts of rational prescribing and strategies for improving prescribing.
* Experience in managing pharmacy services in primary care.
* Experience and an awareness of common acute and chronic conditions that are likely to be seen in general practice.
Desirable
* In-depth therapeutic and clinical knowledge and understanding of the principles of evidence-based healthcare.
* Understanding of the mentorship process.
* An appreciation of the new NHS landscape including the relationships between individual practices, PCNs, and the commissioners.
* Broad knowledge of general practice.
