Knaresborough & Rural Primary Care Network (PCN) is looking for a motivated and aspirational Clinical Pharmacist.
The post holder will be critical in supporting the delivery of the new Primary Care Network Contract Directed Enhanced Services (DES) specifications and will help lead improvements to maximise safe, cost effective best practice to improve the quality of patient care.
This is an exciting opportunity to be part of our well established and experienced pharmacy team working closely with GPs and AHPs across the PCN and the wider locality.
Main duties of the job
The post holder will work within their clinical competencies with ongoing support and supervision from the PCN Clinical Pharmacist as part of a multi-disciplinary team. The role will provide expertise in clinical medicines management, provide face to face structured medication reviews, manage long term conditions, management of medicines on transfer of care and systems for safer prescribing, manage repeat prescription authorisations and reauthorisation, and acute prescription requests for patients.
We offer flexibility for hybrid work arrangements, allowing team members to balance remote work with their personal and professional responsibilities. However, please note that regular on-site presence is essential to maintain the high standard of patient care and effective teamwork required in our GP practices. The post holder would be based across all six practices across the PCN and would be expected to have oversight for the PCN team across all sites.

The role is employed by Yorkshire Health Network on behalf of Knaresborough & Rural Primary Care Network. Yorkshire Health Network (YHN) are the local federation for all 17 GP Practices across Harrogate and Rural areas, containing 4 PCNs in total. YHN and the PCNs work collaboratively to deliver a supportive structure which is focused on achieving the best from a joint management and clinical delivery setup for the benefits of patients across the region.
Job responsibilities
* Work as part of a multi-disciplinary team in a patient facing role to clinically assess and treat patients using their expert knowledge of medicines for specific disease areas.
* Take responsibility for the care management of patients with chronic diseases and undertake clinical medication reviews to proactively manage people with complex polypharmacy.
* Be a prescriber, or be willing to undertake training to become a prescriber, and work with and alongside the general practice team.
* Provide specialist expertise in the use of medicines whilst helping to address both the public health and social care needs of patients at the PCNs practice(s) and to help in tackling inequalities.
* Provide leadership on person-centred medicines optimisation (including ensuring prescribers in the practice conserve antibiotics in line with local antimicrobial stewardship guidance) and quality improvement, whilst contributing to the quality and outcomes framework and enhanced services.
* Through structured medication reviews, support patients to take their medications to get the best from them, reduce waste and promote self-care.
* Have a leadership role in supporting further integration of general practice with the wider healthcare teams (including community and hospital pharmacy) to help improve patient outcomes, ensure better access to healthcare and help manage general practice workload.
* Develop relationships and work closely with other pharmacy professionals across PCNs and the wider health and social care system.
* Take a central role in the clinical aspects of shared care protocols, clinical research with medicines, liaison with specialist pharmacists (including mental health and reduction of inappropriate antipsychotic use in people with learning difficulties), liaison with community pharmacists and anticoagulation.
* To provide support to pharmacist colleagues in the clinical aspects of medicine reviews, clinical audit, practice research and governance.
* Share outcomes of prescribing, clinical, service and patient outcomes audits within the PCN and across the locality in order to drive service and practice improvement.
* To respond to and act on MHRA drug alerts, implementing and communicating any medication changes required as a result.
* To monitor medicines and prescribing expenditure at practice level and propose efficiencies to ensure delivery within budget.
* Be part of a professional clinical network and have access to appropriate clinical supervision.
Person Specification
Experience
* At least 2 years post registration experience in a hospital or a clinical primary care setting.
* In depth therapeutic and clinical knowledge and understanding of principles of evidence based healthcare, medicines optimisation and de-prescribing.
* Appreciation of the nature of GP workload and general practice, NHS Long term plan and PCN DES contract.
* Ability to interpret complex clinical information and data on medicines to deliver face to face clinical care including managing a caseload.
* Experience of working within primary care.
Qualifications
* Mandatory registration with the General Pharmaceutical Council.
* Vocational Masters degree in Pharmacy (MPharm) or equivalent.
* Commitment to become an independent prescriber.
* Specialist knowledge through a postgraduate diploma or equivalent.

£42,000 to £46,000 a year depending on experience.
